Required for all new patients.

Ongoing Medical Care (Monthly)

Hormone medications are prescribed but not dispensed by Hub City Health.
Medical Weight Management

GLP-1 Medication Pricing
GLP-1 medications are dispensed by Hub City Health when clinically appropriate via compounding pharmacy Revive Rx or by prescription. Other peptide therapies may be offered if clinically indicated.
Medication type and dose are determined by medical necessity, tolerance, and response.
Estimated Monthly Medication Cost (dose-medication dependent):

Medication pricing is reviewed with the patient prior to initiation and adjusted only when clinically indicated.
Laboratory Testing

Laboratory services may be billed directly to the patient or arranged at discounted self-pay rates.
Non-Member Services
